Between (A) and (D)...

I will go for 'A' for being more direct and parallel..

(D) - too wordy compared to 'A'

Kindly explain why it is grammatically incorrect.

I guess what I find a little weird in A is "prescribed a drug as a dosage too low ". I would have found "prescribed a drug dosage too low" more aacurate.

I think there is a serious typo in this:

Some patients who do not respond therapies of depression may simply have received inadequate treatment, having, for example, been prescribed a drug at a dosage too low to be effective or having been taken off a drug too soon.

The original typo as threw me off for a few minutes.

If you prefer 'prescribed a drug dosage too low' then it should have been
'prescribed a drug dosage which is too low'

Moreover we should try to stick as close as possible with the original stem in terms of speech and voice...
If we don't find any grammatical errors in (A) we should stick to that.

Is this acceptable..?? :roll:
