POE is the best method here. Also ||ism is a key indicator as well.

Two examples are given:
1. drug prescribed at too low a dosage
2. taken off the drug too soon.

The sentense has to make sense even in the absence of one example.

Now check A -

having, for example, been..., or, having been..

remove the highlighted portion and we are left with - "having, for example, having been" - which does not make sense.

Check C -
for example, having.. , or, being..

remove the highlighted portion now and we are left with - "for example, being taken off.."

Although being is usually not preferred in GMAT, but, in this case doesn't it makes C sound better than A?

In C, is not that ? as & for example mean the samething - redundant

A is correct, though for a moment it deters you from choosing it as the answer because of the Present Perfect Continuous Tense.

why B is wrong..? is it wrong only because the first part has PERFECT TENSE, so the second part should also have perfect tense?

I also chose A.

B/C were eliminated since they had "being"
D needed a conjunction between two sentences
E was wordy and had llism problems.
