I'd rather go for D
D respects parallelism and is clearer about the referent of "it" than A.
In A, it is not clear weather "it" refers to "the presence of a condition" or "a condition", whereas in D, there is no doubt that "a condition" is the referent.
OA?

I would go with D.

Choice C and E out as "Fails to detect when" doesn't make sense.
Choice B inserts "there" which is not needed
Between A and D, A is wordy and has pronoun reference problem (it refers to what: presence or condition).

Left with D as the correct choice.

Medical tests sometimes fail to detect the presence of a condition when it is there and indicate its presence when it is not.

(A) the presence of a condition when it is there and indicate its presence (too many it)
(B) a condition when it is present and indicate that there is one (best)
(C) when a condition is present and indicate that it is present (too many it)
(D) a condition when it is present and indicate that it is present (too many it)
(E) when a condition is present and indicate its presence (too many it)
