D comes closest, IMO.
If you look at the meaning of the sentence, "impose" and "require" should be parallel. "spending, etc" part should modify "doctors". I dont like "and" before "spending", though.

E implies that the subject of "spending" is "insurance plans", which does not make sense.

Among lower- paid workers, union members are less likely than non union members to be enrolled in lower- end insurance plans imposing stricter limits on medical services and requiring doctors to see more patients, and spend less time with each.

a). imposing stricter limits on medical services and requiring doctors to see more patients, and spend
b). imposing stricter limits on medical services , requiring doctors to see more patients, and spending
c). that impose stricter limits on medical services, require doctors to see more patients, and spend
d). that impose stricter limits on medical services and require doctors to see more patients, and spending
e). that impose stricter limits on medical services, requiring doctors to see more patients and spending

Although this question has been discussed several times before, I am still posting because I do not agree with the vox populi.

I think E should be the correct answer because In A and B, "that" is missing. In C and D, the act of "imposition" takes two immediate effects: see more patient and spend less time. E uses "requiring" and "spending" as subordinate of main verb "imposition" and hence should be the answer.

If this question tested modifier and parallelism, test maker would not add "and" before "spending" in D intentionally. Whereas E is flawless.

In E, who do you think is "spending"? E implies that it is "insurance plans" which is a nonsense.

Whatever the case, to see more patients, and spend less time with each are verbs of doctors, so they must be in the parallel structure: to see .. and spend ... after doctors, so B, D and E are definitely flawed. C, as I said, distorts the original meaning

I am making most of the errors in this particular concept.

Thanks in advance!!

IMO 'D' is correct because "insurance plans" impose ... and require ...

spending - is modifying "doctors", addition of "ing" to spend clearly indicates that, and the //ism of the overall sentence is maintained in option 'D'
