C, D, E are awkward.

Between A and B, IMO the answer is B.

Reason:
A: "resistance against pesticides" is ambiguous, and hurts the meaning of the sentence.
B: "resistance ...to pesticides" is more clear.

Choice A can be faulted because it is not at first clear whether caused by modifies increase or malaria. Also, the proper expression here and in choice D is not resistance against but resistance to. Choices C, D, and E are wrong because the completed sentence must read 30- to 4O-fold increase.In these choices, 30- to 4}-fold illogically modifies incidence; furthermore, incidence of . . . increase in D and E is illogical. Choice B is best.

1. In B what you have is not the subordinate conjunction 'because' but the prepositional phrase 'because of '. Hence, there is no need for a clause after 'because of'.
2. All the same, I feel ' because of' is not appropriate here. If you want to use 'because of' then, you should reword the sentence as
- The incidence of Malaria has increased 30 to 40 fold because of … Perse, I feel 'due to' should have been used in the context.

Probably in 1988, some three decades ago, GMAT was not very fastidious about 'due to' or 'because of' .

(B) increase in the incidence of malaria because of increasing resistance of mosquitoes to- Parallel construction. Seems okay. Let's keep it.
(C) increasing malaria incidence because of increasing resistance of mosquitoes to- Increasing is incorrect verb
(D) incidence of malaria increase caused by increasing mosquito resistance against- Same as A.
(E) incidence of malaria increase because of increased mosquito resistance to- malaria increase in not parallel to increased mosquito

So, I think B. :)

I thought 'because' can not be used to provide reasoning support for nouns. Shouldn't we use 'due to' here?
