Among lower- paid workers, union members are less likely than non union members to be enrolled in lower- end insurance plans imposing stricter limits on medical services and requiring doctors to see more patients, and spend less time with each.
a). imposing stricter limits on medical services and requiring doctors to see more patients, and spend imposing and requiring seems to be parallel and it is tempting to select this option, but looking carefully, this sentence have a awkward meaning. b). imposing stricter limits on medical services , requiring doctors to see more patients, and spending awkward meaning. It is not clear what is imposing stricter limits, requiring doctors to see ... c). that impose stricter limits on medical services, require doctors to see more patients, and spend "that" correctly refers to insurance plans but "require doctors to see" is not parallel with the first part d). that impose stricter limits on medical services and require doctors to see more patients, and spending implies that insurance plans require doctors to see more patients. Though that may be true, it is not implied by the original sentence. e). that impose stricter limits on medical services, requiring doctors to see more patients and spendingcorrect. "that" correctly connects the stricter limits on medical servises to the "insurance plan" and "requiring doctors.... " implies that the 'stricter limits' result in doctors to see more patients...
First off, “imposing stricter limits” would be modifying the entire clause preceding the phrase, and this is not correct. The plans are imposing stricter limits, not the fact that union members are less likely to enroll in lower-end plans. This eliminates A & B.
Second, do the plans explicitly require doctors to see more patients? No, the doctors’ seeing more patients is a result of the stricter limits. It is not on the same logical level as the imposition of stricter limits, so this eliminates C & D.

E. Cannot be D
A and B are easily out for wrong verb form - requiring. C implies that insurance plans "spend" less time with each. D is unparalled, unclear and unnecessary "and". E is best for more clarity and parallel.

OA apprearing on brutal SC thread is D. Any insight on this?

The question was posted incorrectly. Look for the highlighted part in D i.e. "and" is not in the question and it makes a big difference. In the above question, D is not possible.
For the following question, D is OA:
