I'll choose A because it sounds idiomattically (is that a word?!?) correct.
Its basically saying that the underwriters want the providers of treatment for mental health problems (aka mental hospitals) to be responsible for ensuring that their treatments are actually benefitting the patients.

I have read that subjunctive mood expresses a wish that a course of action be taken. The typical phrasing is '...that x be y'.
As far as what the SC means it can be read as
The health insurance companies now demand that the providers (hospitals or the institutes that treat mental disorders) be accountable for the treatment and that the treatments have had a tangible benefit (to patients).
Anyone else - pls feel free to correct my understanding.
