A is correct. "so many people were hurt and windows [were] broken...
B. "they" has no antecedent. Original sentence suggests that the ordinance is still in vogue, so "forbidding" is preferable.
C. "were" is needlessly repeated.
D. "had been" is wrong because the "hurt" and "broken" -as implied in the original sentence- occurred - at the same time. "to forbid" changes meaning
E. "had been" wrong as in (D)

For example : The nuts were roasted, and the onions caramelized. (onions were...)
Between A and E
I think the original sentence means that "people were hurt" and "windows were broken" simultaneously sometime in New York. Hence past perfect "had been" is wrong.
