This question is "quirky" in the realm of DS, in that it doesn't fit the traditional 'design' of Value or Yes/no prompts. We're asked which of 3 variables (P, Q and R) is largest. You'll find that TESTing VALUES will help you to quickly prove the insufficiencies involved.
Fact 1: P = Q + 5R + 8
IF....
Q=0
R=0
P=8
Then P is the largest value.
IF...
Q=0
R= -2
P= -2
Then Q is the largest value.
IF....
Q= -9
R=0
P= -1
Then R is the largest value.
Fact 1 is INSUFFICIENT
Fact 2: R = P^2
This tells us nothing about Q. ANY of the variables could be largest.
Fact 2 is INSUFFICIENT.
Combined, we know:
P = Q + 5R + 8
R = P^2
IF...
P=2
R=4
Q= -26
Then R is the largest value
IF...
P=1/2
R=1/4
Q= -8 3/4
Then P is the largest value
Combined, INSUFFICIENT

1) Using the first condition we can get infinite values of p, q and r. Also, we don't know if they are positive or negative.
Since we don't get any UNIQUE values, this condition is insufficient.
2) Using the second condition, we don't know anything about the value of q.
3) Using BOTH the conditions, we get:
p = q + p^2 + 8
Once again, we can have infinite value of p and q.
Hence the answer is E.
