What is the policy around medical history? Can they ask me to provide my medical records? And if so, am I requried to submit those?

I would need more context but in general there is no need for you to present medical records before you are admitted. It is common for some schools to ask for medical records once you have been admitted and have accepted their offer since some US states require proof of immunizations for students even at the graduate level.

I have had health problems that started in high school and continued through my undergrad. I explained the situation in my additional essay as it was a pertinent piece of info in understanding my undergrad performace. I want to know if they usually require the specifics of my medical history upon admittance. And if they require them, i'd like to know if I'm required to disclose them.

The reason why I'm a little concerned is because I haven't been using the same doctors, so not sure if I can get hold of my med history.

The situation you describe is different since you have made your medical history an issue in your candidacy. The schools might ask you to "volunteer" your records to support what you have said in your essay. I do not see how they could literally force you to submit these records but failure to submit them might make them question the veracity of your statements.
