I'm not sure, I answered E myself but after rechecking I think only problem with E that it's plural and that changes the meaning.

(E) hostile work environments, inadequate supports from supervision, and unsafe conditions to work in.

What does "unsafe conditions to work in" mean? "work in WHERE ?"

Remember that "to + verb" is used to express a purpose : "I studied hard to get a good mark" for example; in this example this construct is not used correctly => "and unsafe conditions (in order to) work in" does not make sense.

Can someone please explain why E is incorrect ?

Hi abhi,

Let’s analyze the two choice C and E:

A company’s employee incentive program may become ineffective if its employees experience a hostile work environment, inadequate support from supervision, and working conditions that are unsafe.

(C) a hostile work environment, unsafe working conditions, and inadequate support from supervision.

(E) hostile work environments, inadequate supports from supervision, and unsafe conditions to work in.

Now, it does not make sense to say that employees have several working environments. Every company has its unique working environment, so the expression “hostile work environments” in Choice E is not very logical.

Also “the unsafe working conditions” in Choice C is more concise than the expression “unsafe conditions to work in” in Choice E. When a choice expressions the same idea un a fewer words than the other choices, why not go by that?

For these two reasons, Choice E is inferior to Choice C.

Hope this helps. :)
Thanks.
