I will go with C. If this is not correct than please don't laugh at the explanation :
A) Out of scope. Only one city is being discussed.
B) Seems wrong. Just by hunch C) It means that the new procedures did increased the effeciency and the claims that applaud was false will be proven incorrect by this answer.

I agree with ps_dahiya.
Strunk's efficiency has indeed improved, although strunk is not the most efficient hospital in the city.
Only D damages the conclusion. The argument is based in a comparison between this hostipal and the rest of the hospitals in the city. If the comparasison is not possible, because there is many differences among those hospitals, the argument doesn't have any validity.
