(B) This directly addresses the issue. If veterans have specific health needs that typical hospitals (funded by the VA) are not equipped to handle, it would explain why their health concerns remain unaddressed even though the funding exists. This is a strong explanation.
(C) While this highlights that the problem is particularly severe in this county, it does not explain why the VA plan failed. Not an explanation for the result.

This county has a lot of veterans with health problems. The VA gave millions of dollars to the hospitals to pay for veterans, but this plan didn't work. We have a paradox. What would explain why the veterans' health concerns are not addressed, even though they get their way paid at the hospitals?
(B) is the credited answer. If veterans have special health needs that conventional settings like hospitals don't handle properly, this would explain why the veteran's health concerns have not been addressed.
(A) and (E), rather than resolving the paradox, intensify it. If the veterans have plenty of money for health care (what (A) says) and if the hospitals are successful in treating other people (what (E) says), we would have every reason to think they adequately would address veterans' health concerns. This does not explain why that is not the case.
(C) further describes the intensity of the problem --- the health concerns of veterans in this county have not been properly addressed --- but does not explain why the plan didn't work.
(D) is irrelevant: how money was shuffled around at the federal level is irrelevant to why the hospitals in this county were not able to help these veterans.
