I understand D is the correct answer among others, yet i don't understand whether Aho is referring to "her" or "home". Can anyone explain?

Well, as long as you understand that D is correct, then it's easy.
If Aho had referred to home, there would be no comma needed after a Kiowa matriarch.
In fact, then a better sentence would have been:
In her home 'Aho', a Kiowa matriarch held festivals....

To answer your query, in this context, "Aho" is more likely to be the name of a person - the matriarch: it is of course typical to grant a proper name to a person, but it is relatively rare to do the same for domiciles, so we can reasonably conclude that "Aho" is a person, not the "home".
