Answer choice A is incorrect for idiomatic reasons. You can request X. For example, "Would you like to request a call back?" So in theory, you can also request tests: "I want to request these tests." But if you're going to add another action to the request, which is what this sentence is doing, then the structure needs to change and the subjunctive must be used: "...request THAT x BE y." You don't say "...request x TO BE y." For example, "I would like to request that you give me a call back." See the difference?
