option A has a combination of [simple past and past continuous] which is wrong while Option E has correct usage of Tense [Simple past + Present perfect], then also Option E is wrong

No choice is correct.
In A, B and E, the modifier is wrongly modifying Roger who was not the disconnected one.
In the case of C and D, which are subordinate clauses, the pronoun 'it' refers to the subject 'Roger'.
This author seems to have ignored modification and pronoun norms
