Priyankajaismali In D “it” is referring to the entire phrase “to determine in advance the health care costs for a procedure”. Ask yourself “what is so tricky?”
If we interchange the sequence of the phrases it still makes sense “ to determine in advance the health care costs for a procedure is so tricky that University of New Hampshire has started offering education on health-care literacy.“

Whereas in C “it” is used as a pronoun and we don’t have a singular noun before “it” to which we can logically refer.
