I haven't had many of these, and unfortunately, there's not much I can do about it now. How can I make the best use of this section?
My company is pretty small and does not have any formal reward/recognition systems in place. However, I have been given a few gift certificates (amazon gift certificates) because of performance, as well as an extra week off this year. Is there any way I can communicate this?

I think they have an idea about a lot of industries and how they work. I work for the government and we don't get bonuses, I get awards but they are like what you are talking about and nothing worth writing about.
Just because there are spots for things don't feel you have to fill them. Lots of people will have blank areas.
Hell I dont volunteer for anything at all so try filling up the 5 or 6 spaces they leave for extracurriculars. I am trying to figure out how to work what I do after work with my time into there.

You don't need to volunteer to fill those spaces.
Just do as you said, fill the spaces with what you do with your free time, making sure it makes some sense.
An eg. that doesn't make sense: captain of TV programming and execution at my living room, main responsibilities: choosing shows with the remote.
An eg. that makes sense: organized weekly soccer matches among co-workers. Main responsibilities: programming dates, collecting fees and paying court rentals.
