the sentence starts with because and has a comma before the 2nd clause starts .. in this case, you can't use therefore to start the 2nd clause ....
because he said this, he must be lying [here comma infers that the 2nd clause is a conclusion of the first one]
(D) the investigators were not capable of determining the possible extent of
D changes the meaning
1. how do we know that investigators were NOT CAPABLE [may be didnt have the skill-set] of determining? the original sentence doesn't imply that
2. placement of possible also changes the meaning .. this is a fine point but look closely
possible extend: potential range
possible over-payments: potential over-payments
so the original meaning is range of potential over-payments, NOT the potential range of over-payments
i hope this is clear ... otherwise i'll try to give more examples
(E) the investigators were unable to determine the extent of possible
clear meaning

One more thing to note about D and E. Look at how D is wordier:
(D) the investigators were not capable of determining the possible extent of
(E) the investigators were unable to determine the extent of possible
this simple "concision" change makes the sentence better
