'Had fallen' is not correct, The fall was a later event than the spill occurred C, D, and E are out.
B doesn’t have a subject for the second part of the sentence. 'Amid fears' is a prepositional phrase and not a subject. The intrusion of the connector and the subordinate conjunction 'that' distorts the structure and the meaning of the sentence. A is the correct answer.

There is no harm in skipping the past perfect for the earlier event of the spill because the timeline is clearly marked by the word 'ago."
