A. Seems grammatically correct. I will choose this as the answer. B. “have led” is incorrect since this happened between 1876 and 1904. C. Sounds awkward D. Awkward (“There were … leading” seems incorrect. “serving” seems incorrect as well.”) E. Awkward (“There were … leading” seems incorrect. “for serving” seems incorrect as well.”)
IMHO B Reason 1. I do not like "between 1876 and 1904 " in between construction...of phrase. 2. Concerns still have effect in present so present perfect is also fine.

Wheter its A or B, I have a query.
Why the seperators are not used in any of the option? Like if I use the seperator in A, it will become Concerns about public health led to the construction, between 1876 and 1904, of three separate sewer systems to serve
