For the Stanford application, you have to indicate the contact name and phone number for all of your post-college extracurricular activities. Are they actually going to call them? I'm only asking this, because there was an activity I was involved in over a year ago, where I know that the contact person is no longer at with the organization. I have no idea where he has since moved on to, since I also stopped volunteering there.
I'd include the contact. Given the number of applications they receive, Stanford is not likely to go to that level of detail to confirm a minor claim if they are satisfied with the rest of the application, but the absence of a contact name might cause concern.
If I read your message correctly, the information you are providing is the most accurate up to the time you left the organization/activity. This should suffice unless you view this experience as particularly central to your candidacy.
