Despite working hard, Rachel failed to get a promotion, losing confidence in her abilities, and felt unhappy with her job.

I have doubt about the sentence structure here.

Why there is a comma before the and?
“,and felt unhappy with her job.” - Is this a independent clause? If yes then how? If no why the comma is there?
Can someone please explain
Generally when there are three or more elements in a list, a comma is used before and.

For example:

Peter likes apples, oranges, and mangoes.

Having said that, it is not recommended to choose/reject an option based on such usage of comma.

I noticed there is a parallelism issue. Narrowed down to B and C.

*,verb+ing* triggers cause+effect. Hence chose C.

As per my understanding, I am not sure if *verb(past tense)* triggers the same cause+effect.

Can someone help me?

To answer your query, no; "comma + simple past tense" does not convey a cause-effect relationship; the intended meaning here is that despite working hard, Rachel did three things - failed to get a promotion, lost confidence in her abilities, and felt unhappy with her job; the intended meaning does not convey a sense of causality.
