GMAT 1 - 650 (IR 4, Q 49, V 29)
That was without any preparation.
6 months later with proper preparation
MGMAT 1 - 680 (Q 46, V 37)
MGMAT 2 - 680 (Q 46, V 36)
MGMAT 3 - 690 (Q 47, V 37)
MGMAT 4 - 730 (Q 51, V 39)
MGMAT 5 - 730 (Q 50, V 39)
MGMAT 6 - 730 (Q 51, V 39)
GMATPrep 1 - 760 (Q 51, V 41)
GMATPrep 2 - 740 (Q 50, V 40)
GMATPrep 3 - 740 (Q 51, V 39)
GMATPrep 4 - 740 (Q 50, V 40)
Actual GMAT - 690 (IR 7, Q 50, V 32)
I was shattered to see such a score. I didn't even feel like I was doing badly on the verbal part i.e I got the feeling that I was doing them right as on my mocks.
The mocks were appeared closely mirroring the actual GMAT with 8 minutes breaks (snickers and gatorade) along with AWA and IR.
I slept well the previous day and was cool during the exam.
I am clueless what went wrong with such a pathetic score in verbal section. Any insights would clearly be helpful.

Did you change your verbal strategy when you took the test? Did you have a set plan on how you would handle SC, CR, and RC Qs in addition to timings associated with each?

That's tough man -- especially because the score report doesn't break down how you did in each type of verbal section. But it was clearly a downfall in verbal that brought you under the 700 mark.
If you feel your strategy was airtight, then you should do a re-take and try to get over. It can be emotionally taxing if you do it again and you are STILL under 700. A lot of time could be wasted going a 3rd time. So with that, you should really make sure you got every efficient strategy out there under your belt and ready to execute. Time is valuable to you. You seem like you know what you're doing but you just need enough confidence on test day with whatever frameworks you use to overcome any adrenaline rush or lack of glucose in your bloodstream early in the day when you take the test. Remember to take a light snack with some light carbs so your brain has a chance to stay focus with steady access to glucose.
And of course, try out resources out there and see if there's anything you're missing that could maybe get you over 740 on the real test. I like to recommend trying out the GMAT Pill RC Videos as they've been helpful to many prospective test takers.
