I am trying to find a sample error log to get a good idea of exactly what type of information i should be recording in my error log. I searched the site for a sample and have too many returned results. Can someone please give me an example of the information they are tracking in their error log or provide me with a sample? Should I create an Excel spreadsheet or is a tracking it on paper sufficient?

Mine is very simple.
I have a column for my answer, the right answer, the start time, the stop time, and the difference between times. I use conditional highlighting to highlight any cells where my answer isn't the right answer or cells where the time it took to answer was greater than two minutes.
When I review the questions I've answered, I look to see if I actually did the problem right. If I had the right answer in the right time, then I never look at those again. If I just happened upon the right answer, that question gets highlighted as well. Then everything with a highlight I review weekly until I firmly believe I know how to do it.
