To get a high score in GMAT need to get all the above medium level questions right
This is not true unless you're aiming for a perfect score, at least in the verbal section. I've seen V99% ESRs where the student marked 2-3 easy/medium questions incorrectly. In other words, you can miss some easy/medium questions and still get an exceptional score.
The situation can be different if the algorithm gives the test taker mostly easy questions. Then the impact of even a few errors can be quite large.

but which type of questions of above medium level qns ?
AFAIK, GMAC has never told anyone how it defines "medium" (or "easy", or "hard"). So we don't have access to the item characteristics,
and we don't know how they are used to tag questions as easy/medium/hard.

in layman terms does 25%-45% difficulty level fall in which category ? (is 45% difficulty in GMAT CLUB rating is above medium or just medium ?
GMAT Club ratings are most likely not very precise. Use them as a broad indicator of difficulty level.
Also, why are you looking to map ESR difficulty tags onto GC ratings? I'd advise you to stick to the broad easy/medium/hard categories.
