I was a little surprised to see that "Not Answering The Question" wasn't on the list. Especially when adapting essays from other schools, this can be a major problem. Even when the questions are in the same genre, they often contain subtleties that are glossed over. I'm sure that Adcoms can sense these essays from a mile away and dock the applicant for a potential lack of interest.
