Hi...is there any universal/common format for recommendation, which I can use for all the B school applications?

What do you mean? Are you an applicant or a recommender?
As an applicant, you provide the emails of your recommenders into an online system. The system then sends notifications to the recommenders to provide answers based on some questions. Most schools have different questions, but some have agreed to use the same set of questions, e.g., Harvard, Stanford, Kellogg, ...
As a recommender, you should just follow the instructions given and answer whatever questions they have. There is generally no strict formatting requirement.

I agree with the prior poster's questions and explanations... we do have a Recommendations Guide download available at our store that's quite good and useful if that helps, at https://www.mbamission.com/guides/?category=admissions.
