While taking the online GMAT I encountered a technical issue where I saw the score but was offered a retake if I got a higher score, which I did. So in total I took 1 in person, 1 online with a technical issue and 1 online as a retake.
On applications they ask, "How many times have you taken the GMAT test?". Do I say 2 or 3?

I don't think the scores for the one where you faced a glitch will be kept as a valid score by GMAC as they offered you a retake. So practically you have only two scores I guess.It is better to write 2 and not 3. You can explain why you are not counting the glitched one and you have the support of GMAC as they accepted the glitch and offered retake. But you won't be able to justify why you are counting that as a successful attempt even after GMAT offering retake.

In my opinion, this question is quite vague. You certainly took three official GMAT tests offered by GMAC, but you only took two paid GMAT tests. Can you check your account detail?
