I've just taken the test for the third time and I got 690. The thing is that my quantitative score, which used to be my strength, was only 46 or 66%. That compares with 49 score, or 81%, I took the previous time I took the test. My verbal score reached 38, or 84%. What kind of result would the combination of 49 in quant and 38 in verbal? Should I retake the test in order to bring my score above 700? If I retake the test and receive a lower than 690 score, could it harm my application?

No, there wont be any harm is you score less than 690,since they will take the highest score out of all. Depends if your application round is far then you can.
My personal opinion 690-700 is pretty similar,so unless your target is 720+ you may focus on essays.

I was in a very similar situation to you, my first three scores were 700/670/690. I was determined to break 720+ and managed to do so on my last attempt with some smart studying.
