Scoring so much lower than you usually do has to be puzzling to say the least. However, fortunately, preparing for the GMAT is in a way a pretty straighforward process of learning concepts and developing skills. So, you can certainly sort out what's going on and achieve your score goal.

You've been preparing for three months. Can you give some more detail on how you have prepared? Have you been working topic by topic? Do you practice timed or untimed? Have you prepared for both quant and verbal?
What practice tests did you use when you scored 620 to 700?
What were your section scores (quant and verbal) on your practice tests and on the real GMAT, if you know?
[Hi, I used the GMAT official website, Veritas Prep, Kaplan, etc. for my practice exams. I have been working through analyzing my mistakes after I took it previously and got a 530. During my 530 attempt, I think I was overly stressed, so I ended up having a mental breakdown during the exam. For my most recent attempt, I thought things were going rather smoothly, I did have to guess on a few questions of both Quant and Verbal (3-4), but I had to guess on over 10+ questions each section for my first attempt and still somehow did better. I guess I’m just discouraged and feel that I don’t know what direction to go in as I have exhausted so many practice questions and felt I had a very good understanding of the topics. Even on my first diagnostic practice exam before studying any material I got a 500, so I am more so just defeated with my score. I do not remember my section scores for the 530 because it was cancelled, but I believe for the 440 I got V27 and Q21.]
