I'm going through the same anxiety. I need to talk to my boss about my aspirations and get a good reco from him. I just haven't found a good time. I have a feeling I need to do it this week, good time or not.

If you are planning on a R1 application, you are starting to cut it short. You want to make sure your recommender has enough time to do a good job. Also make sure you provide them with some guidance with how to write a B-school recommendation (its totally different than a business one), why you want an MBA, and what you hope to do post MBA.
