I think you are correctly evaluating your performance and something is not working. It is good you are freaking out, that means you are looking for a solution. I hope the stress and impeding failure will help you look for other options/opportunities to improve your score.
Here are some ideas/questions to get you thinking:
1. This should not have been a surprise. People often don't take full CAT's but they have a sense of how they are doing through target practice. Have you been taking quizzes/questions? were you answering them all correctly?
2. With only 3 weeks of studying, it is quite possible you only covered 20-25% of the material and that's not sufficient to bump up your score

1. No I haven't really. Just maybe 3-4 questions of each topic and wouldn't get them correct. I'm building up quizzes for my performance at the moment, chapter by chapter.
2. I believe I have covered the syllabus since I'm using the
Manhattan Prep books and read everything, plus additional reading from this forum
3. I think this is my biggest mistake, I'm not. I decided that 2 days I will try to learn strategies, instead of solving over and over again since it leads to nowhere.
4. My timing is extremely good which worries me and its because I'm not following the strategies (especially at Verbal).
