The reason many of these top graduate schools accept folks from highly ranked undergraduate universities is because of the strength of the student population. This is just how things work in the world. Universities accept strong candidates from good high schools. Employers recruit from top universities. Top employers recruit (laterally) from other top employers. People go where they think the most, best talent exists.
However, that doesn't mean top talent only exists at top-ranked schools. There are many, many highly qualified applicants who come from lower-ranked colleges. These graduate schools understand this, which is why they accept students from a variety of colleges. As someone coming from a lower-ranked school, you need to demonstrate that you took a challenging course load and did very well, and therefore can go toe-to-toe with students at the high-ranked programs.
Do not feel inferior or underestimate yourself. Put forward a strong application and you will most definitely be given fair consideration!
