It may seem counter-intuitive, but 10000 practice questions is often way too many. Focus on quality not quantity. Make sure as you're redoing questions and then reviewing the solutions, you're paying attention to building consistent processes in each section.

The GMAT is as much about great test taking processes as it is about exhaustive conceptual study and endless practice problems / practice test cycles.

Replaying what happened when you get your score report could be helpful. Perhaps you recall some questions you thought you got correct but, looking back, may have made a careless error on. If you share the score report here, people may provide some specific suggestions as well.
