This question is designed based on Parallelism. Thus , compared things have to be equal in unit. we need phrase here.

D. apparent confusion --> correct parallelism with "extreme lethargy & heightened irritability "

Considering parallel structure, D is the best answer as all the elements are nouns(with adjectives).
