Depends on the school. First, by email. Then, by regular mail. In some cases, they even call you to let you know you are accepted before they release the letters.

If they:
a) can't reach you on phone, or
b) just don't want to call long distance and/or figure out the time difference - which seemed to be the case for me and school X since I received an e-mail from X school telling me they hadn't been able to reach when I had been seating at the office beside my phone all freaking afternoon-.

They may send you an e-mail instead.
So to summarize the order of events seems to be:

1) Phone call and/or personalized e-mail.
2) E-mail with "your decision is ready" link or just check the app. site on D-day.
3) Snail mail package with an admit binder and a school X shirt (or scarf, if it's HBS).

Btw, did you get in? Who were you waiting on? The post is quite old, so I figured you may have had some news by now.
