Shimri from ARINGO here (and LBS alumnus...)

Real bummer about the LOR - BUT, remember, schools receive 1000s of applications and have seen every sort of "technical problem" in the past, and know how to handle them.
(I think 10%-15% of Columbia emails this year to recommenders hit the recommenders Spam folder or didn't reach them. at all.. this stuff happens and schools know how to resolve them)

If you need to resend the LOR request, or change your recommender, check the online application. some schools allow this option. Others, don't and you will have to contact admissions with a good and short explaiantion on why you are switching the recommender
Here is a good example:
"Initially, I chose my direct manager, for the last 6 months, as my recommender, but he recently, he left the company unexpectedly. My previous manager is now my direct manager once again, and I feel it is best to use him as he is the one that recruited me to the firm, and has worked with me directly in the last 2 years"

Also, due to the time of year, if this means you have now missed the school's final deadline, I suggest you ask the school if they will still consider your application, or, is it better to redraw it and resubmit next year
