In option B, aren't we joining a dependent clause using the conjunction 'AND'? Are we allowed to do that?

Yes, this is a valid construction. To clarify, "and" is a parallelism marker, and the phrase "efforts to" has been used as an ellipsis for the second phrase and is used for two things-
1. efforts to move mental patients out of mammoth asylums and
2. efforts to treat them in their local communities
