D & E are out because WHICH should refer to Medicare and here it refers to home health care.
A is not right because "but not with" seems wrong
B is out because "Making benefeciaries" in B is not clear what it modifies.

C it is.. "Medicare covers all costs but not of other non-hospital services for which beneficiaries must pay .... " Clear and consice once all the non-essential things are removed..
A. but not with other nonhospital services where 20 percent of the costs must be paid by beneficiaries. logic for not choosing A - Is where not very awkward in this sentence? Where is used for places.
B. but not of other nonhospital services, making beneficiaries pay 20 percent of the costs. Logic for not choosing B -hospital services cannot make beneficiaries pay money.
