B Is the best answer because, believing, mixing and saying all are happening at the same time, warranting the use of a uniform policy of assigning simple past for all the three events; If we were to use a past perfect for mixing the we have to use a past perfect even for believing , because, the judgment that what she said was fake is the last of the events, the other two happening before that last event. Since no past perfect has been used to describe belief, we can take that the second verb also will be in simple past as in B.

I think D. There are two clauses. In the first clause there are two actions.
1) Mike Believed
2) She Mixed.
These two actions havent taken place at the same time. First she mixed a little truth which made Mike to believe her story. So there should be a past participle with "mixed".
A - "it" has no clear antecedent.
B - Incorrect tense
C - tenses not parallel
D - Past participle used - correct
E - Incorrect tense
