On the last try, I did focus more on Quant and only SC for Verbal. I will give it one more shot, and try to improve my score hopefully to 720. I am not in a rush since I already missed round 1, but definitely want it to be my last time sitting for the exam.
In terms on how I felt on my last exam, Quant was easy, but I did end up really tired at the time I started the Verbal section. I am also considering changing section order to do first verbal. I have read the verbal guides from MGMAT and done OG 2018 and 2015.

Can anyone please suggest me a study plan or materials? I would be really grateful since I put a lot of hours of study but I'm still stuck.

It seems that you have concentrated on your weak areas and have successfully improved in CR and RC. You need to concentrate of SC now. Once you have enough practice, I'm sure your results will soar much higher. Also, ensure that the improved sessions are getting some practice, and not left off completely before the next attempt. You don't want to risk dropping the scores there. Your Quant is good, and I wouldn't suggest changing up the order all of a sudden. If needed, attempt the AWA and IR last, to keep you from getting exhausted. Just concentrate on the weakest area, which is SC for you from the ESR.
