Since that one time appears to be the only time that you've scored above 600, we have to call that an "outlier" and not a pattern. After every CAT, you should be doing a FULL review of every question (including the questions that you got correct). Of course, the immediate attention should be focused on the questions that you got wrong and WHY you got them wrong, but there's something to be said for reviewing the questions that you got correct too. Whatever you did successfully is noteworthy (and will likely be something that you can repeat later).
At this point, your scores seem to place you in, and around, the mid-500s. While that's still a big improvement over your initial scores, there's little evidence to support the idea that you're going to score 650+ if you keep working in the same ways. It might be time to make another adjustment to your studies, invest in some new materials and practice some new tactics. Thankfully, the Round 1 deadlines for most Schools are still over 6 months away, so you have plenty of time to continue studying as needed.

HI Rich,
I always review my questions after the CAT and solve them again without timer and i get most of them right and learn from the wrong ones and go back to it and solve it after a few days.
but what i notice is im not able to see which area is actually my weakness.
Yes i think i should change my study plan ..Can you suggest some studyplan for Quant and Verbal as i have all the sources
(MG MAT guides , OG 12 and 15 , some tough bunch of questions 700-800 level (all topics) , GMAT exam pack 1 and question pack)
so i just a study plan to take my quant up to Q46-47 from Q40 and verbal up to V35-36 from V28-30.
as of now i am giving most of the time to verbal . that 5 days a week to verbal ( monday to wednesday SC - 2 hours everyday) and thurday and friday (CR - 2 hours ) and monday to friday (RC everyday 1 hour after SC and CR)
and saturday i do the math for around 6 hours and more...
and sunday i give a mock..
so i need to plan where i can improve my quant and verbal and at the same time get familiar with mocks giving them
regularly as timing (coupled with test pressure) is one of my major problems.
