1-- I not only caused injury but also caused pain
2-- I caused not only injury but also pain
3-- Not only did I cause injury but I also caused pain
4-- Not only did I cause injury but also pain
5-- Not only did i cause injury but I also did cause pain

When you use not only-- but also first thing you need to make sure is the phrase/clause is balanced. Second thing you want to make sure is that there are no redundancies in the phrase/clause. Lastly you need to make sure idiom not only -- but also makes the sentence consise and clear. Hope this helps

I think 2 is also wrong.. "caused" and "injury" are seperated by "not only". This is wonr usage.
so can i conclude that
Not only xxxxx but [noun]/ [pronoun] also xxxxx
is definitely wrong
Also
" phrase/clause is balanced ", i am assuming xxxx is the phrase clause
What does "phrase should not have redundancy" mean ?
Can you give an example
why is 2 wrong ?
