I have seen a lot of SC questions where an option is wrong because it uses the Passive Voice. Is any option which uses the Passive Voice incorrect? Or is it just less preferred than Active Voice?

Well its not incorrect..I have came across many questions in which the answers are in Passive voice...Its just that Active voice is preferred much more in GMAT.
If you have to choose between a grammattical error and passive voice, go for the latter

If the action is more important than the subject then passive is better - again if everthing else is gramatically correct.
