B. spend more than a billion dollars a year on prescription drugs, but for most patients the medicines have only marginal effects, if any, on symptoms and do nothing

"B" is the right answer because - it corrects the redundant use of "annual"
it ensures a proper "Subject-Verb Agreement" - medicines/do nothing

People in this country annually spend more than a billion dollars a year on prescription drugs, and for most patients the medicines have only marginal effects, if any, on symptoms and does nothing to stop the underlying disease process that eats away at the brain

A. annually spend more than a billion dollars a year on prescription drugs, and for most patients the medicines have only marginal effects, if any, on symptoms and does nothing
B. spend more than a billion dollars a year on prescription drugs, but for most patients the medicines have only marginal effects, if any, on symptoms and do nothing
C. annually spend more than a billion dollars a year on prescription drugs, but for most patients the medicines are having only marginal effects, if any, on symptoms and do nothing
D. annually spend more than a billion dollars on prescription drugs, but for most patients the medicines has only marginal effects, if any, on symptoms and are doing nothing
E. are spending greater than a billion dollars a year on prescription drugs, and for most patients the medicines have only marginal effects, if any, in symptoms and do nothing

Tests Redundancy. Option A and C are out. E is out because of incorrect usage of greater which is used in comparison. Between B and D , B wins because medicines have not has.

E. are spending greater than a billion dollars a year on prescription drugs, and for most patients the medicines have only marginal effects, if any, in symptoms and do nothing

Annually and in an year mean the same thing , thus one of them can be omitted without changing the meaning of the sentence.
A and C are out
D is incorrect because of mismatch between plural medicines and singular verb has
In E greater than a billion dollars in incorrect along with in symptoms

Therefore,
B
