PhD Accounting Prep - What do you think?

Hi,
I'm a senior in accounting and will be graduating this fall from a middle of the road university. I would like to become an accounting PhD, but am definitely behind in the math. I am confident in my GPA and my GMAT abilities, so putting those behind for now, here is my prep schedule:
Graduate in the fall (apply for master's in accounting for next fall)
Spring/Summer - Work at Big 4(I already am CPA eligible)/Take math (Calculus 2, Linear Algebra, ODE) and econ classes (econometrics and hopefully intermediate micro and macro)
Fall 2012 - Start MS program/Continue Working at Big 4/Get CPA
Questions:
Are those enough math classes? Should I take calculus 3 & 4? I'm not looking top 10 here.
Can I start applying for PhD programs in the fall of 2012? I feel my best chances are before December, but I want AdComs to see my graduate grades, which would mean a spring submission.
Any schedule suggestions will be greatly appreciated. I'd like to start my PhD by 2013. If I have to wait a year, so be it, but I'd rather not.
