I'll go with C. It is the only choice that states that it is not ethical to disclose identity of the physician's patient to law-enforcement agencies. 

Other choices merely raise concerns about disclosing the information, but did not state if it is unethical if the information goes to the police.

I also go with C 

the requirements are to report the criminal if they have a gunshot wound or if they have an infectious disease. In this case, the criminal's disease is non-infectious so if the doctor were to report him, he would violate medical ethics

I'd go for C too. 
A- adds to the argument that it would be ethical to report the individual.
B- goes beyond the constraints of the requirements in the argument by stating that it'd be unethical to report anything to authorities.
D- but already gunshot wounds are reported to the law enforcement agencies
E- same as D.
C - clearly agrees with reporting of gunshot wounds as ethical, but states it would be unethical to report anything further to law enforcement agencies specifically, thereby excluding the reporting of certain diseases to health agencies.
