GMAT score: 730 (Q=50, V=38)* Currently employed in Schlumberger as a field engineer. Current job has made me responsible and has taught me team work.
Schools Considered :
1. HAAS
2. MSCF CMU
3. LBS
4. JUDGE
5. MIT
6. Princeton
Query:* Should I give subject GRE math ?
* Should I get reco from workplace or from profs ?

Your GMAT QA score is good enough and thus you dont need to bother about writing a GRE Math test. It will not improve youe chances significantly.
Since, you will be applying to a course, which has a academic inclination, it will be better to have one reco from Work and one from your school prof.
I did the same and was accepted at HAAS MFE for Spring 2010 session.
Nonetheless, for US schools, you will have to show in your essays that your work is quant in nature and exposes you to various finance related challenges etc. Since your target is CMU, you would have to show a good amount of quant work at your workplace(you already have a MTech, so that will take care of the same at academic level).
On the other hand, LBS would like to see more of Finance in your work ex and not quant. It is not a quant school and have a very competitive applicant pool.
