Thank you for your post and for sharing your profile. MBA programs look at your profile wholistically, which means that a less than ideal aspect of your profile can be overlooked if the remainder of your profile is stellar. I think you have a very strong background and interesting experience. Be sure that your personality is tied-in with the remainder of your profile. You mention a failed b-plan and weren't sure if you should mention it. I recommend you do - sometimes failures are stronger than only having successes. Think about what you have learned from the failed experience.
As to your MBA application, it is difficult to ascertain your candidacy without a GMAT score. When are you scheduled to sit in for the exam? I recommend you try and aim for 730+. Also, can you break down your GRE Score (verb/quant)?
